Release of Liability Form
I, hereby acknowledge that I am voluntarily and actively requesting permission for myself and/or my child(ren)/dependent(s), to participate in working with the animals and any other activities on the property at 784 Mount Vernon Road Gap, PA 17527 owned by Desiree & Lamar Stoltzfus and Operated by A Little Love Farm.
I understand that the activity of working with such animals may include dangerous activities involving various risks of injury. I assume the risk for my children, guests, and myself. I further understand that an animal, irrespective of training and usual past behavior and characteristics, may act or react unpredictably at times based upon instinct or fright. This risk is assumed by each participant on the property listed above.
I wish to participate or have my child(ren)/dependent(s) participate in activities recognizing they may be dangerous. I accept and assume the risks of injury (including death) to me, my child(ren), and/or dependent(s).
I further understand that I am responsible for any child(ren), other minor(s), and or any guest(s) who accompany me; and agree that the terms of this release shall be binding as to any other such person(s) or family member(s).
In exchange for being permitted to participate in these activities and accepting the risk assumed associated therewith, for myself, my heirs, guardians and legal representatives, I knowingly and willingly hereby release, indemnify and hold harmless Desiree & Lamar Stoltzfus and A Little Love Farm or Board Members,
employees, contractors, guests, or volunteers, from any and all manner of claims, actions or cause of action of any kind or nature, suits, proceedings, costs, in any way arising out of, resulting from or in any manner connected with my child(ren), dependent(s) or my own participation in the activities.
I further agree not to sue, or otherwise make claims or other demands, against Desiree & Lamar Stoltzfus and A Little Love Farm, or Board Members, employees, contractors, guests and/or volunteers, for any injury, loss or damage resulting from negligence or other acts, and/or omissions, regardless of fault.
This waiver of liability shall be legally binding upon me. I am 21 years of age or older. I have carefully read this waiver of liability, and I understand and fully agree with its contents.
